Understanding Inflammatory Myositis

Inflammatory myositis is a rare condition that causes inflammation in the muscles, leading to muscle weakness, fatigue, and sometimes pain. It happens when the body’s immune system mistakenly attacks healthy muscle tissue. This condition can make everyday tasks like climbing stairs, lifting objects, or even walking more difficult with time. The muscles most commonly affected are those closest to the center of the body, such as the shoulders, hips, and thighs. Dermatomyositis is a type of inflammatory myositis that also causes a distinctive skin rash along with muscle weakness.

While there is no cure,  early intervention and personalized treatment plans can improve muscle strength, manage symptoms, and reduce complications.

Common Symptoms of Inflammatory Myositis

Progressive muscle weakness in the hips, thighs, shoulders, upper arms, and neck

Difficulty climbing stairs, lifting objects, or reaching overhead

Trouble swallowing (dysphagia)

Breathing difficulties if chest muscles are affected

For Dermatomyositis specifically:

Violet or dusky red rash on the face, eyelids, chest, and knuckles

Discoloration and bumps on hands (Gottron papules)

OA symptoms typically develop over time and can vary depending on the affected joint. Symptoms may include:

How Inflammatory Myositis is Diagnosed

Our approach includes obtaining a thorough history and physical exam to start. Blood tests, including muscle enzymes and autoantibodies, can be helpful for diagnosis. We may consider imaging testing such as MRI to evaluate inflammation in larger muscle groups. An EMG to assess muscle electrical activity may also be helpful.

Often, maintenance therapies such as Azathioprine or Methotrexate are needed to help wean patients off of chronic steroids. In more severe cases therapies such as Intravenous Immunoglobulin (IVIG) and biologics such as Rituximab may be needed. We also utilize a holistic approach and include treatments such as physical therapy to maintain strength and flexibility as well as dietary recommendations.
