Understanding Enteropathic Arthritis

Enteropathic arthritis (EnA) is a type of inflammatory arthritis associated with inflammatory bowel disease (IBD), such as Crohn’s disease and ulcerative colitis. It causes joint inflammation and tenderness, affecting the hands, feet, knees, sacroiliac joints and sometimes the spine, alongside digestive issues.

Symptoms of Enteropathic Arthritis

Enteropathic arthritis symptoms often appear alongside IBD symptoms. Common signs include:

Joint pain and inflammation – commonly affecting the knees, ankles, wrists, and sometimes the spine

Morning stiffness – reduced mobility that improves with movement
Swelling, warmth, and tenderness – especially in affected joints
Abdominal pain and frequent diarrhea – due to the underlying bowel disease
Symptoms may flare up in cycles, with joint pain worsening during periods of active IBD.

Causes & Risk Factors

The exact cause of enteropathic arthritis is unknown, but researchers believe it stems from an abnormal immune response. Factors that may contribute to its development include having inflammatory bowel disease and carrying the HLA B27 gene.

Diagnosing Enteropathic Arthritis

Our diagnostic process includes a thorough medical history and physical exam. We obtain blood tests such as inflammation markers and HLA B27 genetic testing. Imaging such as X rays are also helpful for assessing joint damage.
